Instructions:
1. Fill this form out completely. Please do not leave anything blank as all information is essential.
2. Register for Clinic separately from regular swim season.
3. Review the fees and determine whether or not you will be financially responsible for your athlete. Fees are as follows:

*The $56.00 (or $32.00 seasonal) USA Registration fee is not included in fees and is required by the USA Swimming Association.
Note: All returning families are required to participate in a $50 minimum BLAST Fundraiser for the 2007-2008 short course season. This will be reflected in your bill.
** Clinic does not require you to be USA registered.

4. Once you have submitted registration, within two weeks of on-line registration please pay:
Novice, Age Group 1 and Age Group 2: USA Registration fee ($56.00) + 1 month
Age Group 3, Senior 1, Senior 2 and Senior 3: USA Registration fee ($56.00) + 1/2 of season fee.

5. All athletes will need complete their USA registration. Click here to download the form. You may include this form with your payment or return to a coach, or drop it in the payment box at Peet.
